چکیده
زمینه و هدف: اختلال طیف اتیسم یکی از شایع‌ترین اختلالات عصبی-تحولی است که تأثیرات عمیقی بر پویایی خانواده و سلامت روان مراقبان دارد. هدف این مرور نظام‌مند، شناسایی و دسته‌بندی چالش‌های روان‌شناختی و خانوادگی مراقبان کودکان مبتلا به اتیسم و بررسی راهبردهای مقابله‌ای مؤثر است.

روش پژوهش: جستجوی نظام‌مند در پایگاه‌های اطلاعاتی معتبر شامل PubMed، Scopus، Science Direct، Magiran، SID و Google Scholar تا 31 مرداد 1405 (22 آگوست 2026) انجام شد. مقالات مرتبط با معیارهای ورود شامل مطالعات کمی-کیفی، کیفی و مروری در زمینه چالش‌های روان‌شناختی و خانوادگی مراقبان کودکان مبتلا به اتیسم انتخاب شدند. ارزیابی کیفیت روش‌شناختی با استفاده از ابزارهای CASP و AMSTAR صورت گرفت.

یافته‌ها: از میان ۱۲۶ مقاله یافت‌شده در جستجوی اولیه، 11 مقاله مرتبط به طور کامل بررسی شدند. یافته‌ها نشان می‌دهد که مادران به‌عنوان مراقبان اصلی، بالاترین سطح تنیدگی، اضطراب و افسردگی را تجربه می‌کنند. چالش‌های اصلی در شش حوزه دسته‌بندی می‌شوند: (۱) تأخیر در تشخیص و فرایندهای نادرست تشخیصی؛ (۲) دشواری در پذیرش تشخیص و مقابله با علائم رفتاری کودک؛ (۳) دسترسی ضعیف به خدمات درمانی و حمایت‌های اجتماعی؛ (۴) مشکلات مالی و کاهش اشتغال والدین؛ (۵) کمبود فعالیت‌های فراغتی و آموزشی مناسب؛ (۶) داغ‌ننگ اجتماعی و انزوای اجتماعی. راهبردهای مقابله‌ای غالب شامل حمایت‌های مذهبی و معنوی، جستجوی حمایت اجتماعی و استفاده از خدمات تخصصی توانبخشی بود.

نتیجه‌گیری: اختلال طیف اتیسم فشار روانی و عاطفی قابل‌توجهی بر خانواده‌ها، به‌ویژه مادران، وارد می‌کند. طراحی نظام‌های حمایتی جامع، دسترسی به خدمات تخصصی و آموزش راهبردهای مقابله‌ای برای کاهش بار روانی مراقبان و بهبود کیفیت زندگی خانواده‌های دارای کودک اتیسم ضروری است.

Abstract

Background and Aim: Autism spectrum disorder (ASD) is one of the most prevalent neurodevelopmental disorders and has profound effects on family dynamics and caregivers’ mental health. This systematic review aimed to identify and categorize the psychological and family-related challenges experienced by caregivers of children with autism and to examine effective coping strategies.

Methods: A systematic search was conducted in reputable databases, including PubMed, Scopus, ScienceDirect, Magiran, SID, and Google Scholar, up to 31 Mordad 1405 (22 August 2026). Articles meeting the inclusion criteria included quantitative–qualitative, qualitative, and review studies concerning the psychological and family challenges of caregivers of children with autism. Methodological quality was assessed using the CASP and AMSTAR tools.

Results: Of the 126 articles retrieved in the initial search, 11 relevant articles were fully reviewed. The findings indicate that mothers, as primary caregivers, experience the highest levels of stress, anxiety, and depression. The main challenges were categorized into six domains: (1) delayed diagnosis and flawed diagnostic processes; (2) difficulty accepting the diagnosis and coping with the child’s behavioral symptoms; (3) poor access to treatment services and social support; (4) financial problems and reduced parental employment; (5) lack of adequate leisure and educational activities; and (6) social stigma and social isolation. Predominant coping strategies included religious and spiritual support, seeking social support, and use of specialized rehabilitation services.

Conclusion: Autism spectrum disorder imposes considerable psychological and emotional strain on families, particularly mothers. Designing comprehensive support systems, ensuring access to specialized services, and training in coping strategies are essential to reduce caregivers’ psychological burden and improve the quality of life of families with a child with autism.
